# Why Sitting Too Long Hurts Your Back — Body Alignment and Movement Patterns Matter

For many people today, back pain has become an all-too-familiar part of daily life. Many people assume that sitting is a resting activity and therefore places less strain on the back. However, from a biomechanical perspective, sitting places significantly more load on the spine than standing.

When you stand, your body weight is distributed not only through the spine but also across the hips, knees, and ankles throughout the lower body. The spine's natural S-shaped curve — the lumbar lordosis — is also maintained, allowing the intervertebral discs to absorb impact like springs. The moment you sit in a chair, however, this weight-distribution function of the lower body stops, and the entire weight of the upper body becomes concentrated on the lumbar spine and pelvis. If your posture collapses even slightly and your lower back rounds backward, the pressure on the discs can surge to more than twice what it is when standing.

When prolonged sitting becomes a habit, the body undergoes structural changes in order to adapt to that posture. One of the most common postural changes is the shortening and tightening of the iliopsoas and rectus femoris muscles, which are held in a 90-degree flexed position at the hip joint. This makes it difficult to fully straighten up when standing, as the lumbar vertebrae are pulled forcefully, causing pain. A second change is gluteal amnesia — where the gluteal muscles, compressed for extended periods, lose their neuromuscular signaling.

When the gluteal muscles can no longer do their job, the back muscles compensate by taking over, leading to chronic overuse pain. A third change is posterior pelvic tilt and the cascading postural collapse it triggers. As the pelvis tilts backward, the lumbar curve disappears, creating a flat back. To compensate, the thoracic spine rounds into a hunched posture and the head shifts forward, resulting in forward head posture (text neck).

What happens when the hip joints become stiff and the thoracic spine becomes restricted from years of prolonged sitting? When picking something up off the floor, rising from a chair, or rotating the torso, the hip joints and thoracic spine — which should be doing the work — stop moving altogether.

As a result, the lumbar spine, which should remain stable, is forced into excessive twisting and bending as a compensatory mechanism in place of the hips and thoracic spine. Because the lumbar spine is not designed to undergo large rotational and lateral movements, repeated compensation leads to cumulative damage to the lumbar discs and joints — similar to a printing press pressing down repeatedly — eventually developing into chronic pain.

Therefore, to truly address the cause of back pain from prolonged sitting, it is not enough to simply observe an abnormal spinal shape. A functional movement assessment must also be performed — evaluating whether the hip joints flex properly, whether the thoracic spine rotates smoothly, and whether the gluteal muscles activate in the correct sequence when rising from a seated position.
